Weight Management — Single Weekly Care Visit

In-office service. No shipping or take-home medication. Purchase does not schedule an appointment. Request an appointment after purchase; our office confirms availability. Initial evaluation is in person and separately priced. Medication is prescribed and quoted separately after evaluation. No medication dose or escalation is sold as a package feature. Primary-care and MDVIP membership services are excluded. Packages expire one year from the date of purchase and are nontransferable. We prefer 48 hours’ notice for cancellations and require at least 24 hours. Cancellations with less than 24 hours’ notice incur a $50 late cancellation fee. A no-show is charged the full amount for the service or session scheduled that day. We will try to accommodate late arrivals; if we cannot, the appointment may be rescheduled and a $50 late fee may apply. If our clinician determines that you are medically ineligible for a prepaid service, the payment for the service you cannot receive will be refunded.
